Being named as a finalist is testament to the hard work and spirit of collaboration that has embodied this programme, which is:

  • Helping elderly citizens at risk of falls and isolation live safely in their homes for longer
  • Supporting people with mental health challenges to manage their conditions more effectively
  • Enabling younger vulnerable adults to live more independently
  • Reducing preventable A&E transfers, decreasing pressure on NHS services
